Bakteri Tetanus Tetanospasmin
Abstract
Background: Tetanus is an acute toxicosis caused by neurotoxin produced by Clostridium tetani and is characterized by periodic and severe muscle spasms. Tetanus is usually acute and causes tetanospasmin-induced spastic paralysis. Tetanospasmine is a neurotoxin produced by clostridium tetanus. To date, tetanus remains a public health concern in developing countries due to inadequate access to vaccination programs and modern tetanus management including intensive care units for patients with severe tetanus. This disease can affect the nervous system caused by tetanospasmin toxicity produced by bacteria. Tetanus occurs when bacteria enter the body through wounds, insect bites, tooth infections, ear infections, puncture wounds, or cutting the umbilical cord.
Purpose: To discuss aspects of definition, epidemiology, etiology and risk factors, classification, pathophysiology, diagnosis, management, complications and prognosis of Generalized Tetanus.
Method: This type of research uses a qualitative descriptive method with a nursing care case study approach design. This case study is to discuss aspects of definition, epidemiology, etiology and risk factors, classification, pathophysiology, diagnosis, management, complications and prognosis of Generalized Tetanus. This research was conducted at RSUD Jend. Ahmad Yani Metro in September 2023. The case study subject is an observation that must be seen and observed regarding the reality or social phenomena needed in the research.
Results: A 42 year old man came to the emergency room at Jendral Ahmad Yani Metro Hospital accompanied by his family with the main complaint of stiffness throughout the body. The patient has been experiencing this for approximately one week and has gotten worse in the last 2 days. Patients also complain of difficulty opening their mouth, difficulty swallowing, and a stiff stomach. Previous history of seizures was denied. Approximately 2 weeks ago the patient also complained of high fever and chills. The patient denied any previous history of trauma and injury. The patient has a history of cavities and often experiences toothache. There was no history of seizures, headaches, and vomiting. On general examination, the abdomen is tense/stiff, on palpation, abdominal rigidity is found, on motor examination the superior and inferior extremities are difficult to assess. On neurological examination it was found that M. pterygoideus' motor skills were difficult to assess, mouth opening was limited to 1 finger, facial expressions were found to be ricus sardonicus, the position of the uvula was difficult to assess.
Conclusion: Tetanus is an acute toxicosis caused by neurotoxins produced by B. Tetani and is characterized by periodic and severe muscle spasms. Tetanus is usually acute and causes spastic paralysis induced by tetanospasmin, a neurotoxin produced by the tetanus bacillus.

Pendahuluan: Tetanus merupakan toksikosis akut yang disebabkan oleh neurotoksin yang dihasilkan oleh clostridium tetani dan ditandai dengan spasme otot yang periodik dan parah. Tetanus biasanya akut dan menyebabkan kelumpuhan spastik yang diinduksi tetanospasmin. Tetanospasmin adalah racun saraf yang diproduksi oleh clostridium tetanus. Sampai saat ini, tetanus tetap menjadi perhatian kesehatan masyarakat di negara berkembang karena akses yang tidak memadai ke program vaksinasi dan manajemen tetanus modern termasuk unit perawatan intensif untuk pasien dengan tetanus berat. Penyakit ini dapat mempengaruhi sistem saraf yang disebabkan oleh toksis tetanospasmin yang dihasilkan oleh bakteri. Tetanus terjadi ketika bakteri masuk ke dalam tubuh melalui luka, gigitan serangga, infeksi gigi, infeksi telinga, luka tusukan, atau pemotongan tali pusat.
Tujuan: Untuk membahas aspek definisi, epidemiologi, etiologi dan faktor risiko, klasifikasi, patofisiologi, diagnosis, tatalaksana, komplikasi, dan prognosis Tetanus Generalisata.
Metode: Jenis penelitian ini menggunakan metode deskriptif kualitatif dengan desain pendekatan studi kasus asuhan keperawatan. Studi kasus ini adalah untuk membahas aspek definisi, epidemiologi, etiologi dan faktor risiko, klasifikasi, patofisiologi, diagnosis, tatalaksana, komplikasi, dan prognosis Tetanus Generalisata. Penelitian ini dilakukan di RSUD Jend. Ahmad Yani Metro pada bulan September 2023. Subjek studi kasus merupakan suatu pengamatan yang harus dilihat dan diamati mengenai kenyataan atau gejala-gejala sosial yang diperlukan dalam penelitian.
Hasil: Seorang laki-laki usia 42 tahun datang ke IGD RSUD Jendral Ahmad Yani Metro diantar oleh keluarganya dengan keluhan utama kaku seluruh tubuh. Hal ini dialami pasien sejak kurang lebih satu minggu dan semakin memberat sejak 2 hari terakhir. Pasien juga mengeluhkan sulit membuka mulut, sulit menelan, dan perut terasa kaku. Riwayat kejang sebelumnya disangkal. Kurang lebih 2 minggu lalu pasien juga mengeluhkan adanya demam tinggi dan seperti menggigil. Riwayat trauma dan luka sebelumnya disangkal pasien. Pasien memiliki riwayat gigi bolong dan sering mengalami sakit gigi. Riwayat kejang, sakit kepala, dan muntah menyembur tidak ada. Pada pemeriksaan generalis didapatkan inspeksi abdomen tegang/kaku, pada palpasi didapatkan rigiditas perut, pada pemeriksaan motorik ekstremitas superior dan inferior sulit dinilai. Pada pemeriksaan neurologis ditemukan pemeriksaan motorik M. pterygoideus sulit dinilai, membuka mulut terbatas yaitu sebatas 1 jari, mimik wajah ditemukan ricus sardonicus, posisi uvula sulit dinilai.
Simpulan: Tetanus adalah toksikosis akut yang disebabkan oleh neurotoksin yang dihasilkan oleh B.Tetani dan ditandai dengan spasme otot yang periodik dan parah. Tetanus biasanya akut dan menyebabkan kelumpuhan spastik yang diinduksi tetanospasmin suatu racun saraf yang diproduksi oleh basil tetanus.
