Abstract
Background: Difficult intubation is one of the complications of general anesthesia that may lead to ventilation disorders, hypoxia, and other airway-related complications. Pre-anesthetic assessment is essential to predict the possibility of a difficult airway. The Neck Circumference to Thyromental Distance (NC/TMD) ratio and Mallampati score are commonly used methods for predicting difficult intubation.
Purpose: To compare the NC/TMD ratio and the Mallampati score as predictors of difficult intubation in patients undergoing general anesthesia.
Methods: This study employed a quantitative research design using a quasi-experimental approach with a two-group design. The study was conducted at the Central Operating Theatre of PKU Muhammadiyah Hospital Yogyakarta from April 7 to April 22, 2026. A total of 30 respondents were selected using a purposive sampling technique. Measurements included the NC/TMD ratio, Mallampati score, and assessment of intubation difficulty based on the Cormack-Lehane classification. Data were analyzed using the Shapiro-Wilk test and the Mann-Whitney U test.
Results: Most respondents were in the normal BMI category, aged 18–39 years, and classified as ASA II. The majority had an NC/TMD ratio of ≥5.0 and a Mallampati Class III score. The Cormack-Lehane assessment indicated that most respondents experienced moderate to severe intubation difficulty. The Mann-Whitney U test demonstrated a statistically significant difference between the NC/TMD ratio and the Mallampati score in predicting difficult intubation, with a p-value of 0.022 (p < 0.05).
Conclusions: Both the NC/TMD ratio and the Mallampati score have the ability to predict difficult intubation in patients undergoing general anesthesia.
