CPOT ICU Pain Pain Assessment Ventilator

Abstract

Background: Pain is an unpleasant sensory and emotional experience that frequently occurs in critically ill patients in the ICU, including those with decreased consciousness who are unable to verbalize their pain. Therefore, a valid and reliable pain assessment instrument, such as the Critical Care Pain Observation Tool (CPOT), is needed.

Purpose: To report the application of the CPOT to assess pain in ventilated patients in the ICU.

Method: This is a case report of an 80-year-old male patient diagnosed with pneumonia who was hospitalized in the ICU for 8 days. Pain assessment was conducted using the CPOT, which measures facial expression, body movement, presence of the ventilator, vocalization, and muscle tension.

Results: On the first to fifth days, the patient experienced moderate pain (score 3–4), while on the sixth to eighteenth days, it escalated to severe pain (score 6) after tracheostomy placement. Pain responses were reflected in behavioral changes such as grimacing, restlessness, and increased muscle tension.

Conclusion: The CPOT is effective for assessing pain in ventilated patients who are unable to communicate verbally.
