Cadre Competence Health Transformation Integrated Service Post Cadres Primary Service Integration
Abstract
Background: The transformation of Indonesia's health system through Primary Service Integration positions Integrated Health Posts as the spearhead of community-based health services. The shift in Integrated Health Posts functions from solely maternal and child health services to a life-cycle-based service model requires Integrated Health Posts volunteers to possess adequate competence. Basic skills training is one strategy employed to enhance volunteer competence; however, scientific evidence regarding the effectiveness of such training has not yet been comprehensively mapped.
Purpose: To identify, map, and synthesize scientific evidence regarding the impact of basic skills training on the competence of integrated health posts volunteers.
Method: This study employs a scoping review methodology based on Joanna Briggs Institute (JBI) guidelines and is reported in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR). A literature search was conducted using PubMed, MEDLINE (EBSCO), ScienceDirect, and other sources for articles published between 2017 and 2026. Inclusion criteria encompassed full-text articles in English or Indonesian examining training for health volunteers, Integrated Health Posts volunteers, or community health workers, with competence outcomes covering knowledge, attitudes, skills, self-efficacy, or volunteer performance. The selection process was conducted using Covidence. Data were extracted and analyzed using a descriptive-thematic approach.
Results: Ten articles met the inclusion criteria. The synthesis results indicate that basic skills training consistently enhances the competence of integrated service post cadres across knowledge, attitudes, skills, and self-efficacy. Five key themes were identified: improvements in cadre knowledge, attitudes, skills, and self-efficacy, as well as the importance of continuous coaching and supervision. Factors supporting training success include active learning methods, hands-on practice, simulations, mentoring, and support from health workers. Conversely, limitations regarding education, facilities, and a lack of continuous training act as barriers.
Conclusion: Basic skills training for cadres is an effective intervention for enhancing cadre competence. A training model integrating theory, practice, coaching, and continuous supervision is recommended to support the implementation of primary service integration and indonesia's health transformation.
Suggestion: Future research should employ longitudinal or experimental designs with control groups to evaluate the sustainability of the training's impact on cadre competence. Additionally, further research is needed to identify the most effective models of coaching, supervision, and technology-based learning for maintaining integrated service post cadre competence across various public health service contexts.
