Fracture Levine’s Conservation Theory Orthopedic Nursing Physical Mobility

Abstract

Background: Post-operative fracture patients often experience limited physical mobility, which can hinder the recovery process. A nursing approach based on Levine’s Conservation Model focuses on the patient's adaptive capacity through the conservation of energy, as well as structural, personal, and social integrity.

Purpose: To analyze the effect of implementing nursing care based on Levine’s Conservation Model on the mobility of post-operative fracture patients.

Method: A quasi-experimental design with a pretest-posttest control group approach was employed. The sample consisted of 78 participants divided into intervention and control groups using consecutive sampling. Data analysis involved Wilcoxon, Mann-Whitney, Spearman, and Kruskal-Wallis tests, as well as multiple linear regression.

Results: The intervention group showed a decrease in the median physical mobility score from 19 to 11 (p<0.001), while the control group decreased from 20 to 16 (p<0.001). A significant difference was observed between the two groups (mean rank 58.81 vs. 20.19; p<0.001). Age was significantly associated with physical mobility (r=0.233; p=0.040), whereas fracture location showed no significant difference (p=0.530). Regression analysis indicated that the intervention had a significant effect on physical mobility (B= -4.988; p<0.001), while age and fracture location did not have a significant effect.

Conclusion: Nursing care based on Levine’s Conservation Model significantly improves physical mobility in post-operative fracture patients and serves as an effective nursing intervention.

Suggestion: Future studies should involve larger sample sizes, broader research settings, and longer observation periods to evaluate the sustainability of the intervention's impact on participants' physical mobility.
