Evidence-Based Nursing Postprandial Blood Glucose Soleus Push-Ups Type 2 Diabetes Mellitus

Abstract

Background: Three key parameters for managing type 2 diabetes are fasting blood glucose, random blood glucose, and postprandial blood glucose. Each has a distinct role. Fasting blood glucose reflects baseline glucose levels during fasting, while random blood glucose fluctuates due to the influence of various transient factors. Fasting blood glucose reflects the body's response after a meal, specifically related to the body's ability to process glucose insulin according to the mechanism of soleus push-ups (PSPs) performed after meals. This is more relevant in assessing clinical risk and disease progression.

Purpose: To determine the reduction in postprandial blood glucose levels with soleus push-ups in patients with type 2 diabetes mellitus.

Method: Evidence-based nursing (EBN) was implemented using a quasi-experimental pre-test post-test design conducted over three consecutive days. The clinical trial involved 10 hospitalized patients with type 2 diabetes who met the inclusion criteria (stable condition, high postprandial blood glucose, ability to perform ankle movements, and no active diabetic foot complications). The soleus push-up intervention was performed for three days after meals (morning, afternoon, and evening) with a duration of 30 minutes per session and a contraction frequency of 50-100 times per minute. Postprandial blood glucose levels were measured 2 hours after the completion of each daily intervention session.

Results: The average age of the participants was 55-59 years, with a body mass index (BMI) categorized as overweight to obese level 1 (25.0-29.9 kg/m2). The results showed a consistent decrease in postprandial blood glucose levels in all 10 participants, from a pre-test value of 299.4 ± 27.8 mg/dL to 272.4 ± 25.6 mg/dL on day 1, 250.0 ± 23.3 mg/dL on day 2, and a final value of 226.8 ± 21.4 mg/dL on day 3. Cumulatively, the three-day soleus push-up intervention successfully reduced postprandial blood glucose levels by 72.6 mg/dL, equivalent to a 24.2% decrease from baseline. During the implementation period, the intervention was deemed safe with no incidents of hypoglycemia or extreme muscle fatigue.

Conclusion: The soleus push-up intervention was proven safe, effective, and easy to perform, as well as efficient in reducing postprandial blood glucose levels in patients with Type 2 DM within three days.
