Demam Berdarah Dengue Imunoglobulin-M Pasien Suspek Profil Imunoglobulin-G
Abstract
Background: Limited access to and affordability of standard dengue diagnostic tests in primary healthcare facilities in Indonesia result in many cases remaining unconfirmed by laboratory examination. Therefore, routine hematological parameters may serve as a more affordable alternative for early dengue diagnosis, particularly in resource-limited settings.
Purpose: To assess the association and diagnostic accuracy of routine hematological parameters with dengue diagnosis and infection status (primary or secondary) as determined by IgM and IgG serological profiles.
Method: A quantitative study using a cross-sectional approach was conducted from January to December 2024 at the Laboratory of the Pancaran Kasih Evangelical Church General Hospital in Manado. The sampling technique used purposive sampling with a sample size of 141 respondents. The independent variable in this study was suspected dengue fever status, while the dependent variables were the immunoglobulin G (IgG) and immunoglobulin M (IgM) profiles of the patients. Data analysis was performed using R software with a significance level of 0.05.
Results: Platelet count and the hemoconcentration/thrombocytopenia index (HTI) showed the best performance in distinguishing dengue-positive from dengue-negative cases (AUC 0.675), while leukocyte count demonstrated good discriminative ability in differentiating primary and secondary infections (AUC 0.817).
Conclusion: Platelet count and the hemoconcentration/thrombocytopenia index (HTI) are useful as adjunct tools for early dengue diagnosis, while leukocyte count is the best indicator for differentiating primary and secondary infections and risk stratification.
